Q.If I Execute a Living Will or Durable Power of Attorney For Health Care, What do I do With It?

A.You should send copies of the executed documents to your treating physician(s) and hospital(s). Keep a list of the doctors and institutions to whom you have faxed or delivered a copy, in the event you need to contact them (e.g. to revoke or provide new documents). You should also provide a copy to your named agent and discuss in detail your feelings about doctors, hospitals, medication, and treatment.
