Q.Can I Change or Revoke my Living Will or Durable Power Of Attorney For Health Care?

A.YES. You can revoke or change a Living Will or Durable Power Of Attorney For Health Care at any time. To revoke the entire document, including the appointment of your agent, simply destroy it and inform your treating health care provider(s) personally or in writing that you have revoked it. If you want to amend your living will or power of attorney for health care, we suggest you just execute (sign, witness and notarize) a new Living Will or new Durable Power Of Attorney. In such a case, you should personally deliver or fax the new Living Will and/or Power Of Attorney For Heath Care to your treating physician(s).
